Understanding Motivational Interviewing

Motivational Interviewing (MI) is a collaborative, person-centered counseling approach designed to strengthen an individual's own motivation for and commitment to change. Developed by psychologists William Miller and Stephen Rollnick, MI recognizes that ambivalence about change is normal and helps individuals resolve this ambivalence in the direction of positive behavioral change.

Rather than telling clients what to do or confronting them about their substance use, MI therapists create a supportive, non-judgmental environment where clients can explore their own reasons for change. This approach respects client autonomy and recognizes that sustainable change comes from within.

Core Principles of MI

Express empathy through reflective listening and genuine understanding. Develop discrepancy between current behavior and important personal values or goals. Roll with resistance rather than opposing it directly. Support self-efficacy by reinforcing the client's belief in their ability to change.

MI conversations are guided by specific techniques including open-ended questions, affirmations, reflective listening, and summarizing (OARS). These techniques help clients articulate their own reasons for change, which are far more powerful than externally imposed mandates.

MI in Addiction Treatment

MI is widely used across all levels of addiction care, from initial engagement to aftercare. It is particularly effective during the early stages of treatment when motivation may be low or ambivalent. MI can increase treatment engagement, improve retention, and enhance outcomes when combined with other therapeutic approaches such as CBT or 12-step facilitation.
